Sacramental Record Request

If you were baptized, received First Eucharist, were confirmed, or were married at Jesus the Good Shepherd Church, we are happy to provide a copy of your sacramental record.

Please note: Only the individual or their parent/ legal guardian (if the individual is a minor) may request the reissue of a Sacramental Certificate.

Certificates must be picked up in person by the requestor with valid photo identification or mailed directly to a Catholic parish, school, or agency. We cannot mail certificates to personal addresses.

Letters of Good Standing may also be emailed directly to a Catholic parish, school, or agency.

Please allow 10 business days for processing.
